Privacy Policy
This explains what we collect, why, who we share it with, and what you can ask us to do about it. It covers formcoastalclub.com, our booking system, the boutique, and the studio. We are Form Coastal LLC, a Delaware limited liability company. We decide how your information is used, which makes us the controller of it.
Short version: we collect what we need to run a studio and a shop. We do not sell your information. We do not share it with advertisers.
1. WHAT WE COLLECT
You give us:

Payment:
We do not store your card number. Card details go directly to our payment processors, who hold them in tokenized form. We can see the last four digits, the card type, and the amount.

2. WHY WE USE IT

We do not use your information to make automated decisions that significantly affect you. We do not profile you.
3. HEALTH INFORMATION
Some of what you tell us is health information. We treat it carefully and share it only with the instructors who need it to teach you safely. We are not a healthcare provider and we are not covered by HIPAA. That means HIPAA protections do not apply to what you tell us here. We still handle it as sensitive information and will not use it for marketing or share it outside Form except as described in this policy. You can ask us to delete health information from your file at any time. If you do, we may not be able to offer certain sessions safely.
4. COOKIES

5. SMS MESSAGING PRIVACY
We do not share, sell, or provide your mobile phone number or SMS messaging consent data to third parties or affiliates for marketing or promotional purposes. Information may be shared with service providers solely as necessary to provide customer support and operate our services. SMS opt-in data and consent will not be shared with third parties for marketing purposes.
6. MARKETING
Email. You will only receive marketing email if you opted in. Unsubscribe from any message, or email us.
Text messages. We only text you for marketing if you separately agreed. Reply STOP to stop. We keep a record of your consent, including when and how you gave it.
Service messages — booking confirmations, cancellations, payment receipts — are separate. You receive those while you hold an account.
7. CHILDREN
Form is for adults. We do not knowingly collect information from anyone under 16. Guests aged 16 and 17 may attend instructor-led classes with guardian consent, and their information is held under the guardian's account. If you believe we hold information about a child, email us and we will delete it.
